Rhinoplasty

When considering surgery to alter the size or shape of the nose, there are far more options available than ever before. Dr Zacharia can 'customise' the patient's procedure based upon their specific needs, anatomy and desired result. Nasal surgery can be performed to change consequence of genetics, birth defect or injury and can enhance the patient's appearance and/or improve their nasal breathing.

At the initial consultation, Dr Zacharia will ask the patient to express concerns about their appearance and to describe their symptoms. Digital photographs of their face will be taken and Computer Imaging will be used to analyse the nose. This will enable the patient to view their current appearance and determine realistic outcomes following surgery.

Consultation with Dr Zacharia will provide patients with the opportunity to discuss the possible risks and complications of any surgery. This will help them to make a well-informed decision.

Facelift

The facelift is a surgical procedure designed to improve the signs of ageing. The procedure tightens the muscles beneath the skin and the sagging skin itself, while removing fat and improving the appearance of the face overall.

If the patient feels their skin is starting to sag around the jowls and the lower face in general, or if they think they look tired when they are not, then a facelift may be an option. If the sagging is on the forehead or eyebrows, then a brow lift may be a better option.

The procedure takes 2-4 hours and the majority of patients stay overnight in the hospital or surgical facility. Patients will experience initial puffiness and some bruising and swelling which may take several weeks to subside. Most patients are able to resume normal activities within two weeks and begin exercise within 3-4 weeks.

There are many different types of facelifts that can be performed. Some examples are:

* The standard or conventional facelift: This is designed to lift sagging skin and its deeper structures. The incision lines are hidden in the hairline.

* S-Lift: The incision is directly in front of the ears and the layers are moved and pulled laterally. This lift is for the lower third of the face. The jaw line, jowls and neck can be smoothed this way. This is a minimal incision lift and suits patients who have minimal sagging of the skin.

* Lower Facelift: This is the same procedure as a facelift but to the lower face and addresses the jowls and refines the jawline.

* Brow Lift: This is usually done endoscopically via a few small incisions in the hairline. Sometimes an anchoring device, which is absorbed by the body, is used to hold/lift the tissue up. This procedure takes the heaviness and 'hooding' away over the eyes.

During consultation with Dr Zacharia the patient will need to discuss the possible risks and complications of this surgery. This will help them to make a well informed decision.

Chin Augmentation

Chin augmentation, or mentoplasty, is performed using an implant. This procedure can give balance to an otherwise less defined face. It is quite common to have chin augmentation in conjunction with rhinoplasty. Chin implants were originally used for reconstruction, ie. birth defects and trauma related surgery. Currently these implants are used to aesthetically enhance a person's chin or to create symmetry within the facial structure.

Procedure
Surgery is performed using a light, general anaesthetic. This procedure takes from 1 to 1.5 hours, depending on the structure of the patient's face. The incisions are placed as discreetly as possible. The incision can be made either where the mouth gums and lip/facial skin meet, or under the chin. This is a day procedure.

Postoperative Procedure
Patients will experience some swelling and bruising. Many patients can return to work 5 days post-operatively with the use of camouflage cosmetics.

During consultation with Dr Zacharia the patient will need to discuss the possible risks and complications of surgery in order to make a well-informed decision.

Blepharoplasty

Blepharoplasty, or eyelid surgery, is a procedure to correct the sagging or drooping of the eyelids through the removal of excess fat, skin and/or muscle.

Indications
Many people in their early to mid-30s begin to see a noticeable difference in their eyes. They can appear droopier, with bags below and above the eye, sometimes so much that the upper lids appear to disappear under the brow bone. There may also be a puffiness to the eyelids caused by the accumulation of excess skin and/or fat. It is originally under or behind the eye and migrates forward with the ageing of the eyelid tissues.

Prior to surgery
Dr Zacharia will ask the patient to express their concerns about the appearance of their eyes and will discuss their goals and expectations. He will determine if there are any eye conditions that might preclude surgery, and ultimately advise them if eyelid surgery is the best procedure for them.

Anaesthesia
Blepharoplasty is usually performed under a light, general anaesthetic. However, in some cases, local anaesthesia with intravenous sedation may be used.

Procedure
Before anaesthesia is administered, a detailed “map” of the skin and/or fat to be removed is drawn on the skin using a special marking pen. After anaesthesia is given, an incision is made in the curved eyelid crease of the upper eyelid. This placement assures that the scar will be well hidden when it heals. Dr Zacharia will remove the previously determined amount of excess skin and fat, and the incision is then closed with small stitches. In the lower eyelid, the fat is removed through a small incision immediately below the eyelashes. The operation takes one to two hours.

Postoperative procedure
The first one to two hours after surgery are spent in the recovery room where cold compresses are applied to minimise swelling. When the patient is fully alert, they will be able to return home in the care of a family member or friend. The patient will be encouraged to keep their head elevated for several days, and to use cold compresses to reduce bruising and swelling. There is very little if any pain following eyelid surgery. Pain medication is prescribed, although it is not usually necessary. Some patients experience tearing and irritation for the first few days following surgery.

The stitches are removed five to seven days following surgery. The patient may then resume wearing makeup and using contact lenses. The incisions may remain pink for a short period following surgery but can be easily covered using makeup after the stitches have been removed. They are usually completely faded within four to six weeks. Most patients resume work and normal activities after a week.

Results
The goal and result of cosmetic eyelid surgery is a more youthful and well-rested appearance without a change in the shape or expression of the eyes. For many patients these results are permanent.

During consultation with Dr Zacharia you will need to discuss the possible risks and complications of the surgery in order to make a well informed decision.

Otoplasty (Ear Surgery)

Otoplasty or ear surgery is commonly described as 'ear pinning' and refers to the repair of abnormalities of the external portion of the ear. This commonly involves setting ears back closer to the head or reducing the size of unusually large ears. This procedure is usually performed on children aged four to 14 since the ears are almost fully grown by the age of four years old.

When ears protrude noticably, people are often teased by their peers and feel self-conscious. Adults often wear their hair in ways that will hide the ears. Ear surgery can also be performed on adults, without any additional associated risks.

Incisions are placed behind the ears so that there are no visible scars after the operation. If the ears are protruding they may be rotated back and if some of the natural folds are missing they are created. Missing or abnormal sections of the ear may be constructed from tissues taken from other parts of the ear or body.

The operation is performed under a light, general anaesthetic and is a day procedure. The patient is required to wear a pressure dressing after surgery for up to 10 days. Some bruising and swelling may occur.

During consultation with Dr Zacharia it is important for the patient to discuss the possible risks and complications of surgery. This will help them to make a well-informed decision.

 breast

Breast augmentation

Breast augmentation, technically known as augmentation mammoplasty, is a surgical procedure to enhance the size and shape of a woman's breasts.

By inserting an implant behind each breast, surgeons are able to increase a woman's bustline by one or more bra cup sizes. If you're considering breast augmentation, this overview will give you a basic understanding of the procedure – when it can help, how it's performed, and what results you can expect. A lot depends on your individual circumstances. Please ask Dr Zacahria if there is anything you don't understand about the procedure.

Breast augmentation can enhance your appearance and your self-confidence, but it won't necessarily change your looks to match your ideal. If you're physically healthy and realistic in your expectations, you may be a good candidate. Before you decide to have surgery, think carefully about your expectations and discuss them with Dr Zacharia.

A breast implant is a silicone shell filled with either silicone gel or a salt-water solution known as saline.

Breast augmentation is relatively straightforward. But, as with any operation, there are risks associated with surgery and specific complications associated with this procedure, which should be discussed at your consultation.

The method of inserting and positioning your implant will depend on your anatomy, your preferences and Dr Zacharia's recommendation. The incision is usually made in the crease where the breast meets the chest, although it can be made around the areola (the dark skin surrounding the nipple), or in the armpit. Every effort will be made to insure the incision is placed so that resulting scars will be as inconspicuous as possible.

Working through the incision, Dr Zacharia will lift your breast tissue and skin to create a pocket, either directly behind the breast tissue or underneath your chest wall muscle (the pectoral muscle). The implants are then centred beneath your nipples.

You'll want to discuss the pros and cons of these alternatives with Dr Zacahria before surgery to make sure you fully understand the implications of the procedure.

This operation is very successful and has helped many women attain a better figure, which in turn can make an important psychological contribution to their feelings of femininity, confidence, general sense of well being and happiness.

Liposuction

Liposuction is the removal of unwanted lumps, bulges and areas of fat using thin suction tubes (cannulae) that are placed into the body fat through small incisions. A vacuum is applied through a hose attached to the cannula. This draws fat out of the body.

Liposuction is not intended as a weight-loss technique, but is commonly used to remove stubborn fat deposits that are resistant to diet and exercise. Resistant fat deposits, which may be an inherited problem, can be sculpted to create a slimmer profile.

Fat deposits may be formed beneath the chin and on the flanks, hips, abdomen, inner and outer thighs, knees, and lower legs. These are some of the areas where body contouring with liposuction can produce impressive results. The most frequently treated areas in women are the abdomen, hips, thighs and knees. In men, the love handles, abdomen and enlarged male breasts (gynaecomastia) are most often treated. Other areas treated in men include the arms, neck and face.

After liposuction, your new, more balanced and pleasing silhouette should be long lasting and permanent, but you must be prepared to follow an appropriate exercise and eating regime.

Contour Threadlift

Contour Threads are the only non-absorbable barbed suture product cleared by the TGA for the elevation and fixation of midface, brow and neck areas. These specifically designed sutures are made from clear polypropylene, a material that has been safely used in the body for many years in other medical products. In general, the best candidates for a Contour ThreadLift™ include both women and men from the mid thirties to sixties who want a more rejuvenated appearance.

Dr Zacharia makes a small incision behind the hairline and the threads are advanced along the previously marked contours so they create a firm hold on the underlying tissues. The barbs do the work of holding by being fixated to tissue as they are softly contoured into place.

The Contour ThreadLift™ is most commonly performed under local anesthesia and the procedure usually takes less than 30 to 90 minutes to complete, depending on the number of threads used. This is a day procedure. The patient will experience minor bruising, swelling and possible dimpling and may be able to return to work 2-3 days after the procedure, however it could take up to 7 days. Depending on the age of the patient at the time of the procedure, the degree of sagging and the number of threads used, the duration of the effect varies from two to five years.

Contour ThreadLift™ does not replace a conventional facelift; rather it offers patients an innovative, minimally invasive surgical approach to achieving facelift-like results.
